Step 3: Complete Your Bank of Commerce Credit Card Requirements

To get a Bank of Commerce credit card, you need to provide the following documents:

  • Accomplished Bank of Commerce Credit Card Application Form[1]
  • Photocopy of any valid government-issued, photo-bearing ID. Here’s a list of acceptable IDs: 
    • Passport
    • Driver’s license
    • Company ID
    • Voter’s ID
    • TIN ID
    • SSS ID
    • Postal ID
    • PRC ID
    • Student ID (issued and signed by the school head or principal for the current school year)
    • Other government-issued IDs

  • Latest Certificate of Employment and Compensation
  • Latest Income Tax Return (ITR) or BIR Form 2316
  • Payslips for the last three months

📌 For Self-Employed Applicants:

  • Copy of business papers
  • For single proprietorships – Certificate of Registration of Business Name issued by the DTI
  • For partnerships – Articles of Partnership and Certificate of Registration issued by the SEC
  • For corporations – Latest Articles of Incorporation (AOI), By-Laws (BL) and Certificate of Registration issued by the SEC. Furthermore, you’ll need to submit the Certificate of Filing of AOI and BL, Board Resolution, or Sec. Certificate, and List of Elected Officers
  • Latest ITR for the last two years stamped as received by the BIR or authorized bank
  • Latest Audited Financial Statement (AFS) for the last two years stamped as received by the the BIR or authorized bank
  • Last six months’ bank statements

  • Certificate of Employment and Compensation
